COVID-19 Vaccine Clinic Appointments

Online appointment booking for COVID-19 vaccine clinics is currently open for the following populations*:
*All non-aged-based groups must be 18 years-of-age and over.

  • Phase 2 groups:
    • Individuals aged 40 years and older (i.e., born in 1981 or earlier)
    • Residents, their essential caregivers and frontline workers in high-risk congregate living settings
    • "Highest Risk", "High Risk" and "At Risk" health conditions under the Province's Primary Priority for Phase Two (PDF), as well as the applicable essential caregiver (for those with the "Highest Risk" conditions).
      Note: All pregnant individuals are eligible to receive vaccine under the "Highest Risk" health conditions group. Please note, partners of pregnant individuals do not qualify to receive vaccine under the "essential caregiver" designation. 
    • First group of essential workers who cannot work from home:
      • Critical event response
      • Enforcement, inspection and compliance
      • Childcare workers
      • Foster care agency workers
      • Agriculture and farm workers
      • Food manufacturers
      • Funeral directors and cemeteries
      • Elementary/secondary school workers, including educators, custodial and administrative staff
    • Second group of essential workers who cannot work from home:
      • Essential and critical retail workers (including grocery, foodbank, pharmacy, ServiceOntario, postal service, ServiceCanada, Passport Canada, wholesalers and general goods, restaurant, LCBO workers)
      • Workers in manufacturing industries directly involved in supporting the COVID-19 response, constructions (including infrastructure) and other essential businesses and services where facilities are at heightened risk for COVID-19 outbreaks and spread
      • Social workers and social services staff who provide in-person client services
      • Courts and justice system workers (including probation and parole workers)
      • Transportation, warehousing and distribution workers (including public transit, truck drivers supporting essential services, marine and rail cargo and maintenance, highway maintenance)
      • Electricity
      • Communications infrastructure workers
      • Water and wastewater management workers
      • Financial services workers (bank branch staff)
      • Veterinarians and veterinary teams
      • Waste management workers
      • Oil and petroleum workers
      • Natural gas and propane gas workers
      • Mine workers
      • Uranium processing workers
    • Staff, essential caregivers and any residents that have not yet received a first dose in:
      • Long-term care homes
      • High-risk retirement homes
      • First Nations elder care homes
    • Alternative level of care patients in hospitals who have a confirmed admission to a long-term care home, retirement home or other congregate care home for seniors
    • Health care workers identified as highest priority, followed by very high priority, in the Ministry of Health's guidance on Health Care Worker Prioritization (PDF)
    • Adults ages 80 and older
    • Staff, residents and caregivers in retirement homes and other congregate care settings for seniors (for example, assisted living)
    • Health care workers identified in the Ministry of Health's guidance on Health Care Worker Prioritization (PDF)
    • Indigenous adults
    • Adult recipients of chronic home care

Vaccination clinics are by appointment only. No walk-ins will be accepted at clinic locations.

Please do not book if you are not part of one of the above eligible groups. Eligibility will be checked on your appointment day.

On your appointment day, you will be required to bring your health card (OHIP) or any government issued-photo ID (if you do not have an OHIP card) to confirm your appointment and age (if booking as 50 years of age or over).


We strongly recommend online booking. If you are unable to book online, please ask a family member, loved one or friend for assistance. If you are unable to book online, please call 519-753-4937 and press "2" to access our booking line.

Please leave a message with your name and phone number. Someone from our team will call you back to book an appointment. Please have a valid health card or other alternative type of government issued identification ready for the individual(s) you are booking an appointment for. As we have a high volume of calls, we appreciate your patience.

For those not included in one of the current eligible groups listed above, the Province expects remaining eligible Ontarians who wish to be vaccinated will be able to do so beginning in July 2021 and beyond.
