Johnson & Johnson (J&J) COVID-19 vaccine

Please note: If you have previously completed the below form and have not yet received a vaccine, you do not need to re-register.

The Government of Ontario has secured a very limited supply of the single dose Johnson & Johnson (J&J) COVID-19 vaccine as part of its efforts to vaccinate as many eligible individuals as possible in the province.

The J&J is a non-mRNA vaccine that is available to individuals aged 18 and over who:

  • - have not yet received any doses of COVID-19 vaccine, wish to receive the J&J vaccine and do not have a contraindication to it. Please note the J&J vaccine should only be used when an mRNA vaccine (Pfizer, Moderna) is declined and after informed consent.
  • - experienced an allergic reaction to a previous dose of an mRNA COVID-19 vaccine (Pfizer, Moderna) and/or other contraindications to an mRNA vaccination (excluding myocarditis or pericarditis). Please note a referral from a healthcare specialist may be required on the appointment day.

If you fall within these groups and are interested, we ask that you pre-register using the form below.

The J&J vaccine will be administered at a BCHU clinic within Brant and clients will need to be available at a date and time determined by BCHU. Clients will be contacted by BCHU in advance with further details on where and when to receive the vaccine. This will be on a first come first serve basis whiles supplies last. BCHU cannot guarantee a J&J vaccine to all those who express interest. Please note due to limited supply, the J&J vaccine available at BCHU clinics is available for Brant residents only.

Prior to pre-registration, please review the J&J information sheet at: Janssen COVID-19 Vaccine Information Sheet (gov.on.ca)
