​COVID-19 Vaccines

Third dose booking:
Please review your vaccination receipt before booking a third dose appointment. Eligible populations 18 years-of-age or older (i.e., born in 2004 or earlier) must wait a minimum of 12 weeks (84 days) after their second dose was administered before obtaining a third dose. Those who do not meet the 12 week (84 day) period between second and third doses will not receive a vaccine at their appointment time. Please note: adults must be at least 18 years-of-age on the appointment date to receive a booster vaccine.

5 to 11-year-old vaccinations:
Children aged 5-11 may receive their first or *second dose by appointment or through one of our walk-in clinics. You may book an appointment using the booking form below. Please note: children must be at least five years-of-age on the appointment date to receive a first dose vaccine.

*Second doses are available at our walk-in clinics only for those eligible eight weeks after receiving a first dose.

For children that may require extra time or attention during vaccination, we recommend booking appointments at our family friendly clinic at 195 Henry St., Brantford (located beside the Ministry of Community Social Services, Ontario Disability Program office).      

Walk-ins:
Walk-ins are being accepted for first, *second and **third dose COVID-19 vaccinations for those 5 years-of-age and older at our clinics. Individuals looking to obtain a third dose are required to make an appointment using the below online booking tool. Please note: children must be at least five years-of-age on the appointment date to receive a first dose vaccine.
*Second doses are available at our walk-in clinics only for those eligible after eight weeks.
**Adults 60 years-of-age and older (i.e., born in 1962 or earlier) and pregnant women are eligible to receive a third dose of COVID-19 vaccine by walk-in, no appointment necessary. Please see our walk-in clinic schedule for more information.

COVID-19 vaccine appointment booking form

Online booking is available for the following populations:

  • First and/or second dose appointment bookings for all individuals born in 2017* or earlier 

  • Please note: children must be at least five years-of-age on the appointment date to receive a first dose vaccine.

  • Third or booster dose appointments for individuals who received their second dose of the COVID-19 vaccine at least 12 weeks (84 days) ago including:

    • Adults 18 years-of-age or older (i.e., born in 2004 or earlier). Adults must be at least 18 years-of-age on the appointment date to receive a booster vaccine.

    • Healthcare workers and designated essential caregivers (click here for eligibility information)

    • First Nations, Inuit and Metis adults, including non-indigenous household members (click here for eligibility information)

    • Individuals who received a complete series of a viral vector vaccine (i.e., two does of AstraZeneca or one dose of the Janssen vaccine)

  • Third dose appointments for all eligible moderately to severely immunocompromised individuals  who received their second dose of the COVID-19 vaccine at least 8 weeks (56 days) ago. Click here for eligibility information.

  • Fourth dose appointments for all eligible moderately to severely immunocompromised individuals who received their third dose of the COVID-19 vaccine at least 12 weeks (84 days) ago. Click here for eligibility information.

Eligibility will be checked during the booking process and on your appointment day. Those who do not meet the eligibility criteria will not receive the vaccine.


Eligible individuals may also book their COVID-19 vaccine appointment through select pharmacies and primary care settings participating in the vaccine rollout.

We strongly recommend online booking. If you are unable to book online, please ask a family member, loved one or friend for assistance. Call volumes are currently high. If you are calling regarding appointment dates, please be aware that appointments currently displayed on our online booking portal are the first available appointments being offered. If you are unable to book online, please call 519-753-4937 to access our booking line.

More clinic appointment dates will open for booking as we continue the vaccination campaign. If clinic dates to meet your eligibility needs are not yet available for booking, please to continue to monitor our website for future clinic information.
